Welcome to the Dignity Health artwork order request form. This tool will allow you to easily initiate a new artwork request for your Dignity Health facility.

Please allow two business days for a response.
For each new artwork request, your art consultant will assist you in selecting the appropriate imagery and determine the proper piece count based on your budget and the design standards set by Dignity Health. ARCH Design maintains a record of the artwork at each Dignity Health facility to ensure there are no duplicate pieces of art.
